The Dorset General Practice Alliance is helping local health teams work together to improve people's health and wellbeing, tackle unfair health differences and bring care closer to home.
The Clinical Commissioning Local Improvement Plan (CCLIP) brings together general practice, community pharmacies, community groups and other local organisations. By working together, they can find new ways to support people and help local people live healthier lives.
Local teams are developing projects that focus on the things that matter most to their communities. These include helping older people stay independent and avoid falls, supporting children and young people's wellbeing, helping people living with dementia and improving care for people with learning disabilities.
The Alliance is helping by providing advice and support, organising review panels, sharing learning and celebrating good ideas from across Dorset. As new projects get underway, we will share stories about the people and partnerships making a difference in local communities.
Tim Goodson, Chief Executive Officer of the Alliance, said, 'GP practices are at the heart of local communities. Every day, practice teams work hard to help people stay well and get the support they need.
'This programme gives local teams the chance to build on that work, try new ideas and work more closely with partners. We are proud to support these projects and look forward to sharing the positive difference they make for people across Dorset.'
