Dorset’s Neighbourhood Health and Wellbeing programme

People are living longer, often with multiple long-term conditions, frailty, or complex needs and a system that mainly responds when people become unwell is no longer sustainable.

We need to prevent people from reaching a crisis point and to support them earlier.

Dorset’s Neighbourhood Health and Wellbeing programme is aiming to streamline access to health in the county, helping people stay well for longer with more proactive, personalised care.
