Clinical Commissioning Local Improvement Plan
Helping Dorset communities live healthier lives
Across Dorset, general practice is working with local partners to improve people's health, tackle unfair health differences and bring care closer to home.
The Clinical Commissioning Local Improvement Plan (CCLIP) helps make this happen.
Through CCLIP, Primary Care Networks (PCNs), community organisations, pharmacies, councils and health services can work together on projects that make a real difference to local people.
The Dorset General Practice Alliance is proud to support this work.
What is CCLIP?
CCLIP is a Dorset-wide programme that helps local teams:
Improve people's health and wellbeing
Reduce health inequalities
Support neighbourhood working
Develop new and innovative services
Improve care for local communities
The programme is made up of three parts:
Prescribing Improvement for Neighbourhood Health (PINH)
Supporting better use of medicines and helping people get the most from their treatment.
Innovation Fund
Supporting projects that improve cardiovascular, renal and metabolic (CVRM) health and reduce health inequalities.
Transformation Fund
Supporting local teams to design new services around the needs of their communities.
Celebrating general practice
General practice sits at the heart of neighbourhood health.
CCLIP gives practices and PCNs the opportunity to work with partners across Dorset to create new ways of supporting patients.
From community-based clinics and prevention programmes to wellbeing services and innovative partnership projects, local teams are helping improve lives every day.
Examples of the types of projects

-
Exploring new ways to improve support for people living with dementia.
-
Developing new approaches to support children's and young people's mental wellbeing
-
Improving access to services and helping reduce health inequalities for people with learning disabilities.
-
Creating respiratory clinics that bring patients and professionals together for joined-up care.
-
Identifying and supporting people at greater risk of cardiovascular, renal and metabolic conditions, helping to improve outcomes and reduce health inequalities.
Supporting CCLIP work
-
The Dorset General Practice Alliance helps coordinate and support the delivery of CCLIP across Dorset.
We work alongside the Dorset ICB, Primary Care Networks (PCNs) and neighbourhood partners to:
Provide guidance and support to applicants
Manage the application and assessment process
Organise CCLIP panels and reviews
Coordinate programme communications and learning
Share examples of good practice across Dorset
Support project monitoring and reporting requirements
Our role is to help practices and partners navigate the process and make the most of the opportunities available through CCLIP.
The Alliance does not make funding decisions or guarantee approval of applications.
-
A key part of CCLIP is bringing organisations together to improve health and wellbeing in local communities.
The programme encourages Primary Care Networks (PCNs) to work alongside:
General practice
Community pharmacy
Local Authorities
Voluntary and community organisations
Patient groups
Neighbourhood partners
Many projects are designed and delivered jointly, helping local organisations build on existing relationships and respond to the needs of their communities.
Smaller PCNs are encouraged to work with neighbouring PCNs and local partners to share skills, resources and expertise.
-
CCLIP assessment panels are currently planned for:
22 October 2026
5 November 2026
19 November 2026
26 November 2026
Projects are expected to be in delivery by early 2027.
